Adopt a Bouvier

TO A PROSPECTIVE FRIEND OF THE BOUVIER: Thank you for expressing your interest in the Bouvier des Flandres for adoption. The Bouviers offered have been screened and are believed to be desirable dogs. We hope to provide them with excellent homes. For each Bouvier adopted, we ask reimbursement for actual expenses incurred. Completing and submitting this questionnaire can help us become better acquainted, and aid us in determining a "match". The info you provide remains confidential within our program unless you give permission on this questionnaire to distribute your info with other rescue organizations or appropriate places on the internet.

This program is run by volunteers and donations, so if you don't hear from us when you expect to, please feel free to check in at any time. You can follow up on your submission by contacting Judy Kasper at 951.780.0419.  This questionnaire applies only to residents in Southern California, and does not guarantee receipt of a Bouvier. 

1)  Download file. 

2)  FILL OUT the application ON YOUR COMPUTER.

3)  When finished, save the file as a PDF with your name in the title and click on the "submit" button on the last page.  It will affix to your email client as an attachment to Judy Kasper 

OR

4)  Print and FAX (or Snail Mail) the completed form to Judy Kasper.
